Getting Started
Follow this link and click on "HBW Contract and AIG Appointment". Print the paperwork. If unable to reach someone call the BAI Administrative Assistant at 865-607-0424.
Important things to note:
- Make sure you have the latest forms from the HBW website.
- It is always best to use a credit or debit card and fax everything back to HBW.
- When in doubt leave parts blank, our licensing and contracting department will fill in for you and if they need to call for information they will.
- Complete the APPLICANT INFORMATION section on the top of the Associate Application as completely as possible.
- In the section that says to be completed by Recruiter be sure that your recruiter’s name is correct. If you don’t know the rest of the information ask your recruiter or just leave it blank. My number, Dan Brandfast is 1185.
- Be sure to sign and complete the payment section on page 3 of the Associate Application.
- Complete blocks 1 & 3 of the New Agent Information Form. (Ok to put recruiter’s name in block 2)
- Complete the AIG APPOINTMENT APPLICATION. Start by checking LBG on top and complete either the INDIVIDUAL, CORPORATION, or both if an officer of the corporation.
- Answer all questions on the BACKGROUND INFORMATION and give full details of any “YES” answers.
- Complete LICENSING AND STATE APPOINTMENT REQUEST by putting your home state and also any other states you wish to be licensed in, assuming you are already non-resident licensed.
- Fill in your name and sign where it says, signature on the bottom of page 2 APPOINTMENT APPLICATION. You should leave the RECRUITER PAGE blank for the home office to fill out.
- On the AGENCY AGREEMENT enter your name on top, SSN or TIN and sign as Representative.
- Complete the LIFE INSURANCE SOLICITOR SALES AGREEMENT as instructed on the page.
- On the Direct Deposit Authorization:
- Put Social Security Number and your name in the top section.
- Fully complete the second section.
- Sign and date the third section.
- Be sure to fax your E&O coverage certificate with the application (if you have one).
- Sign and print name on the second page of the Supplement for Prepayment of Overwrite Commission form.
- Upon completion, fax back to the number on the cover sheet with a copy of state licenses for all states you wish to be appointed in. YOU SHOULD ONLY FAX BACK THE FORMS MENTIONED HERE.
- Next step is to get with your upline trainer to establish a business plan that coincides with your goals and that will enable you to reach those goals.
